Abstract
Introduction: Non-typhoidal Salmonella bacteremia is an uncommon condition, primarily associated with immunosuppression. In cancer patients, it can rapidly progress to sepsis and septic shock, with high mortality if timely treatment is not initiated. Case Presentation: We describe the case of a 50-year-old woman with a history of squamous cell carcinoma of the anal canal and systemic lupus erythematosus. She had a diverting ileostomy and was admitted to the intensive care unit with gastrointestinal sepsis and septic shock. Blood cultures showed growth of Salmonella spp. sensitive to beta-lactams and fluoroquinolones. Treatment with meropenem was initiated for 7 days, resulting in a favorable clinical and microbiological response. Discussion: Invasive non-typhoidal Salmonella infection should be suspected in immunocompromised patients with fever and gastrointestinal symptoms. Etiological identification through culture and early administration of targeted antibiotics are essential to improve prognosis. Conclusion: This case highlights the importance of considering Salmonella spp. as a possible causative agent of sepsis and septic shock in patients with cancer and immunosuppression, especially in contexts of foodborne or environmental exposure.
